Value of 18-FDG PET/CT in assessment of solitary pulmonary nodule

Abstract


FDG-PET/CT providespowerfulinformationontumor characteristics –bothmorphologicandfunctional. Inadditionto providingaccurate functional/anatomiccorrelation,theCTscanclearly detectssmallerpulmonary metastasesthatarebelowtheresolutionof PET.

18 FDG PET/CT helps characterize SPN, as most of malignantnodulesshow increasedglucosemetabolism. Thediagnostic accuracy of18FDGPETdependsonthesizeofthenoduleandits avidity for18FDG,andfalse-negativestudieshavebeenreportedin nodulessmaller than1cm, well-differentiatedadenocarcinoma, bronchio-alveolar cellcarcinoma,andcarcinoid.

False-positivefindingsinclude infectious and inflammatory processessuchas tuberculosis,fungalinfections,andsarcoidosis.It has beenreported that 18 FDGPET/CTcan reliablycharacterize SPN morethan7mm providingvaluableinformationtoguidepatient management, especially useful where biopsy is risky, in elderly patients, orwhenthere isalow riskformalignancy. This, SNPswith increased18FDGuptakearelikely malignantandshouldundergo further invasive resection orbiopsy. However, nodulesPETnegative stillneedtobefollowed(usually by CT)becauseofthepossibilityof false-negative PETfinding.



PET/CT offersanumberofadvantagesinradiationtherapy, including reducedriskofgeographic misses;aminimizeddoseof ionizingradiationtonon-targetorgans,definitionof radiation treatment field oftumorinatelectasis.Alsoitplaysamajorroleinfollowupof malignant tumorsand their response to therapy.
