THE EVALUATION OF CORD BLOOD HAPTOGLOBIN AS A BIOMARKER OF EARLY ONEST NEONATAL SEPSIS

Abstract
Neonatal sepsis is a blood infection that occurs in an infant younger than 90 days old. Sepsis is a severe illness in which the blood stream is overwhelmed by bacteria. It is a clinical syndrome that complicates severe infection and is characterized by systemic inflammation and widespread tissue injury and complicated by acute organ dysfunction.
Haptoglobin is an acute phase reactant,it is a plasma glycoprotien synthesized predominantly by the hepatocytes. Haptoglobin binds free hemoglobin during hemolysis preventing oxidative change by reactive oxygen species and reducing iron loss through urinary excretion.
The Haptoglobin-hemoglobin complex is cleared after binding to CD163 receptor on monocyte and macrophages The antioxident &immunomodulatory effects of Hpare relevant in hemolytic and infection Diseases, Diabetes Mellitus and renal and coronary arteries.
This study included 60 neonates, group 1 (proved sepsis clinically and laboratory. group 2 30 neonates were suspected to have neonatal sepsis clinically but laboratory free.
All patients in the study were subjected to adequate history taking, full clinical examination, CBC,CRP, blood culture and cord blood haptoglobin assay.
• In our study we found that neonates with neonatal sepsis had their cord blood haptoglobin higher than those of the non septic group.
• There was a significant positive correlation between haptoglobin level in cases with CRP and blood culture.
• Our study revealed that sensitivity of cord blood Haptoglobin 13.33 % Specificity of 100%, positive predictive value of 100%, negative predictive value of 53.37% with diagnostic accuracy of 56.67%
• So further studies are needed to define the diagnostic value of cord blood haptoglobin in early onset neonatal sepsis.
