Predicting Pelvic Adhesions in Women with Endometriomas by Combining Three-Dimensional Vaginal Ultrasound and Serum CA 125

Abstract


The purpose of this study is to assess the accuracy of combining three-dimensional transvaginal ultrasound scan based markers with serum levels of CA125; in the detection of pelvic adhesions in women with endometriomas.
The current study conducted on 132 women with a clinically suspected endometriotic cyst, diagnosed by 2D ultrasonography scheduled for lapraroscopy, found that 3D transvaginal ultrasonography (blurring ovarian margin markers), CA125 serum level (cutoff value 75 IU/ml) and both in predicting pelvic adhesions in endometriotic patients having sensitivity and specificity 85- 100%, 6.9% -89%, 5.7%-100% respectively. Predictive value of positive = 100% and predictive value of negative = 35.4% for both.
We concluded that combining 3D transvaginal ultrasonography and CA 125 serum level has a very low sensitivity of only 5.7% and 100% specificity which it means loss of most of cases to be diagnosed preoperatively which was the aim of this screening test for effective patient triaging for offering patient best care by
