Abstract


Trichiasis is the sight-threatening consequence of conjunctivalscarring in trachoma, the most common infectious cause of blindness worldwide.(6) Trachomatous trichiasis is the result of recurrent infections from childhood with C. trachomatis,which causes chronic inflammation in the tarsal conjunctiva.This produces conjunctival scarring, entropion, trichiasis, andultimately blinding corneal opacification. The disease causes painful, usually irreversible sight loss.(51) Over seven million peoplehave trachomatous trichiasis world wide,(43) mostly those living in poor rural communities in 51 endemic countries.(44)
Blindness due trachoma is irreversible once it has occurred, but it can be prevented. The SAFE strategy (Surgery for trichiasis, Antibiotics to treat C. trachomatis infection, Facial cleanliness and Environmental improvement to reduce transmission of C. trachomatis from one person to another) is recommended by the WHO for the control of trachoma. With the SAFE strategy, the WHO is targeting the Global Elimination of Trachoma as a cause of blindness by the year 2020 (GET 2020), one element of a broader strategy known as (VISION 2020): "The Right to Sight" (12)
Surgery isthe mainstay of treatment and is often effective. A multitude of surgical procedures have been devised and tried.For mild cicatricial upper lid entropion with adequate lid closure, anterior lamellar reposition andbilamellar tarsal rotation have been advocated assuccessful procedures.(67)
