USE OF CO2 LASER LAPAROSCOP IN TREATMENT OF PELVIC PATHOLOGY LEADING TO INFERTILITY

Abstract


Experience and adequate training are the most important factors for t he success of these o peration and decrease of the morbidity and mortality rate. Laparoscopic t raining should include animal laboratory ex perience to learn the essentials of biophysics of equipment and their applications.

C02 l aser remai ns the most precise laser for treatn1ent of various st ages of endometriosis. With COlaser. w hat you see is w hat you get because it is ver y superficial in penet ration. There is no heat abso rbed by adjacent t issues and so there is no post operati ve swe lling as compared to
electrosurgery. To t al pregnancy rate in patients with endometriosis was 32% (7/25) in Stage I, 28% (7/26) in Stage II and 20CJc (8/40) in Stage III. Most of the preg nancies in e ndometriosis patients occurred within the ftrst t\\•elve months after surgery. AFS scoring system for endometriosis does not match with the severit y of the symptoms and also with the outcome after treatment. New methods for classification of endometriosis is the cornerstone for good assessment of patients with endometriosis related infertili ty.

Concept ions of patients wit h hyd rosal pinx treated by laparoscopy depe nds on the co nd ition of the tube. Select ion of the patients are mand atory to get better results. Preg nancy outcome after surgery u sually occur witrun the first year after surgery. After that the cumulative pregnancy rate will be plateaued. Patients who do not get pregnant after one year should be shifted to Assisted Reproductive Technology. Total pregnancy rate in this grou p was 14.9% (6/41). 83% (5/6) was full term , 17% (1/6) were ectopic pregnancies.

Pregnancy rate after salpingo-ovariolysis is comparable to those obtained by controlled patients. During any pel vic surgery, good and meticulous hemost asis, copious irrigation, using the microsurgical principles and addition of materials to decrease the incidence of adhesions are all mandator y to avoid these complications. Total pregnancy rate was 37.8% (17/45). 76.4% (13/17) was full t erm, 17.6% (3/17) was a bortion , with 6% (1/17) ectopic preg nancy. Two thirds of the pregnancies occurred in the ftrst year after surgery and the rest of preg nancies occu rred between the second and the third year.
