Abstract


The study included 20 patients (9men and 11 women), their age ranged between 16-29 years and weight between 43-63 kg. Patients were randomly allocated into two groups, 10 patients each. The autotransfusion group who received autologous blood transfusion by cell saver device and the control group who received homologous blood transfusion. All patients had valvular heart diseases, 10 patients had l'vfitral valve replacement and the other 10 had double valve replacement (l'vfitral and Aortic). Patients who had a previous cardiac operation, hepatic or renal impairment, evidence • of drug or disease induced iminunosuppression, history of previous blood transfusion were not included in the study. The anesthetic and CPB were standarized for all patients. Blood samples were obtained for measurement of hematocrit, platelet count, leukocytes and their differential counts, CD4, CD8, CD45
RA and CD14 .aLthe following times: just before CPB, just after CPB and

24h. after CPB. The complete blood picture was done and the differential counts were determined by Abbott Cell Dyne-3600. Monoclonal antibodies were .used for identification of lymphocyte subpopulations (CD4, CD8, CD45 RA and CD14).

In both groups, the total leucocytic count showed insignificant increase at the end of CPB but there was significant increase 24h. after CPB. The neutrophil count showed insignificant increase at the end of CPB but there was significant increase 24h. after CPB which was more in the control group. There was insignificant decrease of total number of lymphocytes at the end of CPB but there was significant decrease at 24h. after the bypass.
