Abstract


Anatomic development of the hypothalamic-pituitary­

thyroid system occurs during the first trimester of gestation.

The placenta is freely permeable to iodide but it imposes a relative barrier to the thyroid hormones and is impermeable to TSH.
The ontogeny of mature thyroid function involves

both hypothalamic pituitary thyroid gland ontogenesis and maturation as well as the maturation of each of the component systems for thyroid hormone action.
Hypothyroxinemia of prematurity means low levels of bound and free plasma concentrations ofT4 and T3 in the preterm infants.
Hypothyroxinemia of prematurity IS caused by a

combination of factors such as immaturity ofhypothalmo­ pituitary-thyroid axis, sudden loss of materno-fetal transfer ofT4 and effects of neonatal disease (non thyroidal illness).
