Assessment of the Oblique Sandwich osteotomy for Augmentation of Posterior Atrophic Mandible

Abstract


This study was conducted on ten patients selected from the outpatient clinics of the Oral and Maxillofacial Surgery Departments of Faculty of Oral and Dental Medicine Ain-Shams University, and Misr International University.
The aim of this study was to evaluate the alveolar bone volume by the CBCT after performing oblique sandwich osteotomy in conjunction with interpositional ramal autogenous bone grafting in the atrophied posterior mandible. Preoperative measurements were taken these measurements were the baseline for the comparison of the results of the augmentation procedure and for follow up after 4 months.
Radiographic stents were fabricated with radiopaque markings then all patients were scanned preoperatively while the patients wearing the stent.
The osteotomy was performed using the microsaw in the buccal aspect of the ridge with oblique direction. Then the full bucco-lingual osteotomy was completed using ridge splitting chisels. The coronal bone fragment was carefully mobilized by rotation and elevation.
Autogenous bone was harvested, milled and packed under the raised osteotomized segment. The osteotomized segment was fixed by using miniplates and screws. Platelet rich fibrin membrane was prepared and placed over the grafted area. Then the flap was repositioned and sutured.

The cases were followed and evaluated clinically at 1 week, 1 month, 2 months, and 4 months post-operatively. Also, CBCT measurements were obtained at the 4 months follow up. All the gathered records were tabulated and fed into the computer to obtain statistical analysis of the cases.
