UMBILICAL RECONSTRUCTION: A REVIEW OF TECHNIQUES

Abstract


To understand today's patient who seeks body change, we must understand the history of humans, the attitudes they had towards their bodies and the way in which they tried to change them.
The economic status of the environment is often reflected in the dress as well as in the body form of people. plastic surgery offered a more permanent changes for body reshaping for people who are looking for ideal body shape.
The umbilicus is an important feature of the abdomen .It is a key aesthetic landmark of the anterior abdominal wall and its absence or distortion a frequent cause for concern and patient complaint.
The umbilicus is a fibrous cicatrix covered by adherent puckered skin lies a little below the midpoint of linea alba.The normal position of the umbilicus has been described as occupying the midline, level with the iliac crests and approximately 2.5 cm. It has dual arterial blood supply ( from deep layer and skin ) and can survive on either one of them.
Venous drainage of the paraumbilical region had two kinds of pathways to a deep vein: through the superficial inferior epigastric vein to the femoral vein and through the vena comitans to the deep inferior epigastric vein .
Massiha et al describes variation of the appearance of the umbilicus with different body types in his paper for umbilicoplasty. The aesthetic pattern of umbilicus is different shape throughout human history . Umbilicus shapes were classified into four categories (chanel logo , bell-shaped, oval and almond shape .
The ideal umbilicus is apparently small, oval and non protruberant . It has ring shape with, tubular wall, base and superior hooding with vertical or T-shaped orientation. A distorted or horizontally orientated umbilicus has been deemed less attractive by both surgeons and the patient.
The umbilicus may require surgical reconstruction due to congenital or acquired conditions . congenital condition include congenital hernia,bladder exstrophy, gastroschisis, omphalocoele, cloacalexstrophy . Acquired condition include inflammatory destruction (necrotising fasciitis, umbilical sepsis, clostridial myonecrosis). Surgical procedures (umbilical herniorrhaphy, abdominoplasty, laparotomy) or excision for skin tumors .
