Total Aortic Occlusion" A Clinico-Pathological Study

Abstract


Thirty three patients with totalaortic occlusion have been divided into tow groups, a prospective(13 patients} and a retrospective(20 patients} group. Risk factors in the retrospective group includes, diabetes mellitus in 5 patients, ischaemic heart disease in 7 patients, hypertension in 2 patients, smoking in 11 patients, and cerebral manifestations in one patient. In the prospective group, the risk factors were diabetes mellitus in 10 patients, ischaemic heart disease in 8 patients, hypertension in 4 patients, smoking in
18 patients,hyperlipidaemia in 2 patients, and cerebral manifestations in 5 patients.
The presentations of the patients have ranged between disabling claudication, rest pain,tissue loss and sexual dysfunction. Angiogram was performed to all patients.
Surgical interventions have been individualised to each patient however, end-to-side juxtarenal aortic anastomosis was performed in all patients using Dacron graft. Renal endarterectomy,femoro-popliteal bypass, profundoplasty, and toe amputations were added according to the indications. Complication rates were within the reported international figures. Histopathology results in both groups were similarly reported as an atherosclerotic calcified plaques.
