Mediolateral VS Lateral Episiotomies in Primparus Patient; A randomized Controlled Trial

Abstract


Background: The precise origin of episiotomy is unknown. Description appeared in European texts by 1740's. In the eighteenth century, episiotomy was introduced as an obstetric procedure where a surgical incision is to be made in to the perineum to widen the vaginal opening for delivery.
Objective: Comparison between mediolateral and lateral episiotomy in primiparous patients.
Methodology: This study was done at Ain Shams University Maternity Hospital in the period from November 2016 up to July 2017. This study was done to compare between mediolateral and lateral episiotomies. Six hundred patients collected, group I is three hundred thirty- four patients candid for episiotomy and group II is two hundred sixty-six patients need no episiotomy. Group I is subdivided in to two groups according to type of episiotomy (mediolateral and lateral) each group is of one hundred sixty-seven patients.
Results: The current study was conducted at Ain Shams University Maternity Hospital during the period between November 2016 and July 2017. 334 primiparous patients were included in this trial divided in to two equal groups each group is of 167 patients.
Conclusion: By comparing lateral episiotomy and mediolateral episiotomy, duration of repair in mediolateral episiotomy is shorter than that in lateral episiotomy and number of threads used in lateral episiotomy is more than that used in mediolateral episiotomy. mediolateral episiotomy had less complication than lateral episiotomy such as hematoma, vaginal and wound infection. Pain in mediolateral episiotomy is less than that in lateral episiotomy and this was calculated by visual analogue scale.
Recommendations: So, we recommend that the use of mediolateral episiotomy is much better than using lateral episiotomy as lateral episiotomy has more complication, more threads used in its repair and longer duration of repair.
