Brain Natriuretic Peptide as an Indicator of Fluid Overload in Hemodialysis Patients

Abstract


Although the cardiac biomarker B-type natriuretic peptide (BNP) is strongly related to mortality in end-stage kidney disease (ESKD), whether it is a predictor of weight change or blood pressure (BP) response upon probing dry weight among hemodialysis patients remains unknown. The purpose of this study was to examine among hemodialysis patients whether BNP is a biomarker of excess volume.
BNP is 32-amino acid polypeptide secreted by the ventricles of the heart in response to excessive stretching of heart muscle cells. Its biologic effect is to decrease in systemic vascular resistance and central venous pressure as well as an increase in natriuresis. Thus, the net effect of BNP is a decrease in blood volume.
In this cross-sectional study we included forty patients with ESKD, without cardiac diseases, who were subjected to regular dialysis treatment three times a week for the duration of four hours. Our study included 40 patients (26 males, 14 females), seven patients were diabetic, nineteen patients were hypertensive. Seventeen patients were smokers The average age was 43.175±12.322 years (ranging from 22 to 70 years) .the mean body mass index was (25.77 ± 2.57) Kg/m². The average dialysis time was (24.850) months,mean Collapsibility index was 29.248 % measured by echocardiograpy .The mean systolic blood pressure was 140.38 ± 26.224 mmHg, and the mean diastolic blood pressure was 81±13.737 mmHg . Blood chemistries and BNP were collected after dialysis.
The aim of the study was to find a cut off value for BNP in hemodialysis patients with fair cardiac function which indicates concealed volume overload that need revision of postdialysis dry weight to preserve cardiac function & reduce the risk of cardiovascular mortality on the long term.
In this study there was non significant difference between hpervolemic and normovolemic patients as regard patient's characteristics including the gender of patient, smoking, presence of DM or hypertension or as regard laboratory investigations.
