Auto uterine transplantation: an experimental clinical trial in female dog model

Abstract


nfertility due to a lack of anatomical uterus or functional uterus had eluded reproductive medicine for a long period of time. Uterine factor infertility is estimated to affect thousands of women worldwide and can be caused by either congenital Müllerian malformations, such as in MRKH syndrome, or more commonly acquired as in the cases of Asherman’s syndrome, myomas or surgical absence of a uterus after a hysterectomy. Human uterus transplantation is a potential future treatment of AUFI.
In the last decade, Uterus transplantation models have since been developed in several animal spe¬cies starting with rats, mice, rabbits, pigs, sheep, and non human primates have been used as experimental models, and pregnancies were achieved in some of these before entering the experimental clinical stage of human.
To date, thirteen human uterine transplantation attempts had been done worldwide. In 2000, the first human UTx attempt was made in Saudi Arabia but failed due to rejection. The second human UTx was performed in 2011 in Turkey, using a deceased nulliparous donor but the recipient had yet to bear a child after at least two early miscarriages and multiple further IVF attempts.
The first clinical series of uterus transplantation cases was performed in Sweden in 2012/2013 by Professor Mats Brännström and Swedish team. Nine uterine transplants in women with uteri from live donors had been involved. During the first 4 months, 2 of the women had hysterectomy, in one case due to thrombosis of the uterine arteries and in another case due to a severe untreatable intrauterine infection. The remaining seven women started menstruating during the first 4–8 weeks post surgery, and the grafts remained viable with regular menstruations during the post transplantation years.
In 2014, the first live birth after UTx was announced then reported three further live births with one ongoing pregnancy. In 2015, a trial of human uterus transplantation was performed in China with successful transplantation. In February 2016, another trial was performed in Cleveland clinic in USA
