Thyroid Function Tests in relation to Glomerular Filtration Rate in patients with Chronic Kidney Disease

Abstract


Subclinical primary hypothyroidism is highly prevalent in the general population, especially in the elderly. However, the prevalence of subclinical primary hypothyroidism in persons with chronic kidney disease (CKD) not requiring chronic dialysis is not well defined. The presence of subclinical hypothyroidism in patients with CKD might be a risk factor for both cardiovascular disease and progressive kidney disease.

The aim of this work is to study the correlation between the thyroid functions and the glomerular filtration rate in patients with chronic kidney disease.

A cross sectional multicentered study of 200 outpatients with chronic kidney disease with estimated GFR less than 60 ml/min was done, the study group was from different centers including Ain Shams University hospital, El Ganzoury specialized hospital and El Khanka central hospital, the study group included 200 patients classified into 3 groups according to estimated GFR using cockroft gault formula (50 patients in Stage 3 chronic kidney disease, 50 patients in Stage 4 chronic kidney disease and 100 Patients in Stage 5 chronic kidney disease). A full history was taken from patients including sociodemographic status (age, sex, special habits of medical importance, family history and co-morbid conditions). Laboratory data were collected including: serum urea, serum creatinine, serum uric acid, hemoglobin level, Serum TSH, FT3 and FT4 concentrations.

Data of 200 patients were collected, their mean age was 50.61±10.09 years (range between 24 and 72 years). Males versus females were (47% Vs 53%). Smokers were (23.5%) and chronic analgesic abusers were (2%).

As regards the co-morbid conditions, hypertension was found in (35.5%), diabetes mellitus (21.5%), ischemic heart disease (14.5%), chronic liver disease (12.5%) and (23%) of patients had no co-morbidities.
