The Prevelance of Food Allergy in Adult Patients with Gastro-Esophageal Reflux Disease

Abstract
Gastro-esophageal reflux disease (GERD) is a highly prevalent gastrointestinal (GI) disorder and is one of the most common GI illnesses encountered in clinical practice.
Heartburn and acid regurgitation are the most common symptoms reported by patients with GERD. Food allergy can manifest as cutaneous, respiratory, systemic (anaphylaxis) or gastrointestinal (GI) symptoms.
The aim of this study was to determine the prevalence of food allergy in adult patients with GERD of unknown etiology.
This was a cohort study performed on 50 adult patients with GERD attending the gastroenterology clinics of Ain-shams university hospitals and referred to the allergy clinic of Ain-shams university hospitals for completion of work up for diagnosis.
Patients were subjected to full detailed history, clinical examination, GERD and allergy questionnaires, gastroduodenscopy with multiple biopsies taken from different parts of esophagus, PH manometry for some patients, food challenge test, skin test and patch test for the essential food allergens.
Out of 50 patients; 56% were females and 44% were males. Their mean age was 30.78±7.87. Most of the patients were grade A (66%); and 28% were grade B and 6% were grade C.
As regards the investigations of food allergy; 70% of the patients were SPT positive. And out of the 15 SPT negative patients, only one patient was patch test positive.
Out of the 36 GERD patients with food allergy(72%), 97.2% had improved GERD symptoms after diet elimination either with medications (71.4%) or without medications (28.6%). Meanwhile only one patient (2.8%) didn’t improve on diet elimination.
There were insignificant correlations between the grades of GERD and each of the socio-demographic distribution, the age of onset of GERD and the history of food allergy among the studied patients.
In addition there were insignificant correlations between the grades of GERD with the socio-demographic distribution and the age of onset of GERD among patients with GERD and food allergy.
