LAPAROSCOPY OF THE UPPER URINARY TRACT

Mohamed M. Nabil El Ghoneimy;

Abstract

In the last few years, laparoscopy has evolved to become a powerful tool in the urologic
surgical armen,tarium. Almost all urologic procedures can now be perfonned laparoscopically with varying re'sults and acceptance by the urologic community.

The use oflaparoscopy in upper urinary tract surgeries spares the patient the morbidity of a large lumbar •incision; thereby offering him a speedy and smooth postoperative recovery with
rapid return to n' ormal activity. This is clearly the major benefit of laparoscopic surgery.

As rega'rds operative and postoperative parameters, although the improvement in video technology and instrumentation has pushed laparoscopy to almost the same square as open surgery, yet thee are still certain limitations regarding the selection of patients and the learning
curve.

To this oment yet, recurrent nonfunctioning kidneys or kidneys with prior episodes of severe inflammation pose a certain difficulty even to experienced laparoscopists. Size criteria are still an important issue limiting laparoscopic radical nephrectomy to small confined renal masses. Laparoscopic partial nephrectomy is still a procedure of expertise requiring equipped centers with
a long laparoscopy learning curve.

Other simple prOcedures such a cyst deroofing and nephropexy have been proven to be easier and less invasive usig laparoscopy rather then open surgery.

LaparosCopy has been identified as a procedure that requires dedication, specialization and a long learning curve. However, once properly mastered, experienced laparoscopists can perform several procedures as efficient and as precise as open surgery with even better operative and postoperativoutcomes.
