OPEN EXCISION VERSUS EXCISION WITH TRANSPOSITION RHOMBOID FLAP IN MANAGEMENT OF CHRONIC PILONIDAL SINUS

Abstract


Pilonidal sinus is a serious condition that is often d1fficult to treat, the treatment is coasty in terms of surgical procedures, medications, in­ patient stay, and post-operative wound care.

This is a prospective randomized clinical trial study tb•evaluate the results of excision with duformental transposition rhomboid flap repair of chronic pilonidal sinus versus open excision technique. The study was carried out on 36 patients with chronic pilonidal, 27 males (75%) and 9 females (25%) with 3:1 ratio. Their ages ranged between 17 and 35 years
with a mean age of 24± 2 years. It's more common .111 the 3'' Jccack of
life (83.4%). The mean duration of the illness before surgery was 7.5± 1.8 months. There was no statistical significant difference between the two groups regarding sex and age distribution, duration of illness before surgery, sinus opening, past history of previous infection. past historv of prev1ous operation for pilonidal sinus, hair distribution and family history.

Transposition rhomboid flap consume more operative time 13 5±9.4 minutes than open excision operation 42±5.1 minutes.

Patients were followed up for a mean period of 7.6±2.2 months at the out patient clinic. Fourteen patients (77.8%) with transposition rhomboid flap were free of pain at the end of the 1'' week. While most patients (88.9%) in the open excision group, had pain of cliff'crcnt degree at the end of 1 '' week postoperative. The difference between the two groups was statistically significant.
