TRANS VAGINAL US ASSESSMENT OF PREMALIGNANT AND MALIGNANT CHANGES IN ENDOMETRUIM IN POSTMENOPAUSAL BLEEDING

Abstract


Transvaginal ultrasound is known to be a goodtechnique of screening for endometrial lesions. TVUS in patients with postmenopausal bleeding improves diagnostic accuracy and clinical decision making.
Endometrial thickness of 4 mm or 5 mm or less almost completely exclude endometrial carcinoma.
In cases of thickened endometrium, endometrial hyperplasia and/or carcinoma should be considered.
According to the TVUS finding there areseveral sonographic features that help todiscriminate benign from malignant conditions:
 Homogenous regular and hyperechogenic endometrium with small cysts within are indicative of benign process.
 Heterogeneous echopattern with irregular hypoechoic areas and interruption of endometrial halo were more in favor of malignant processes. In addition to Sonographic ability in detection of myometrial encroachment.
Transvaginal colour Dopplerand power Doppler mapping are noninvasive methods that have a significant role in the diagnostic procedures to evaluate of PMB. Transvaginal colour Doppler can help todiscriminate benign from malignant endometrial changes.
Lastly, we may conclude that transvaginal sonography is an excellent screening tool for detection of endometrial abnormalities and could be considered as the first imaging modality for diagnosis of endometrial lesions.
