SHORT AND INTERMEDIATE TERMREGISTRY OF PERIPHERALARTERIAL INTERVENTION

Abstract


Endovascular treatment for occlusive disease has revolutionized the treatment of peripheral arterial disease, Act as a less invasive revascularization procedure andhas been proven to be an effective technique for the treatment of PAD.
Currently, the primary indications for interventional procedure in patients with peripheral arterial disease affecting the lower limbs are:
1- Life-style Limiting Claudication and/or Claudication that limit the ability to earn a living.
2- Critical Limb Ischemia.
Stents have become the default strategy in most endovascular interventions because of the excellent immediate results and durable long-term patency.
This study was designed to accomplish the following:
Evaluate the immediate, short and intermediate term outcomes of percutaneous trans-luminal interventions for the treatment of Peripheral Arterial Disease (PAD).
Our study enrolled 27 patients who underwent PTA ± Stenting for lower extremity atherosclerotic arterial disease in 31 limbs. The indication for intervention were lifestyle limiting claudication in 18 (58%) limbs, chronic critical limb ischemia in 12 (39%) limbs and acute critical limb ischemia in 1 (3%) limb.
Anatomic success (residual stenosis <30%) was achieved in 31 (100%) limbs. And overall Initial Technical Success was achieved in 31 (100%) limbs.
Major procedural complications were reported in 4 (15%) patients in the form of death, the first patient died one day post-procedure due to uncontrolled hemorrhage from puncture site, the second patient died 3 days post-intervention due to disseminated intravascular coagulopathy, and heart failure, the third patient died on the 9th day of intervention from septicemia after he underwent below knee amputation, the fourth patient died 3 weeks after intervention due to myocardial infarction and hemodynamic collapse.
Follow up was performed at 1 and 6 months. With clinical success rate at one month 96.2% and at six month was 95.7%.
Clinical status at last follow up visit revealed sustained improvement in 22 limbs (95.7%).
