EFFECTS OF ABDOMINAL OPENING ON RESPIRATORY MECHANICS, HAEMODY NAMIC AND SOME HORMONAL AND METABOLIC VARIABLES DURING GENERAL ANESTHESIA IN NORMAL AND MORBIDLY OBESE PATIENTS: A COMPARATIVE STUDY

Abstract


This study was caiTied out on 30 patients of both sex, their age ranged between 20-56 years, their body weight ranged from 50-190 kilograms, their height ranged from 1.50-1.90 m. and their body mass index (BMI)
ranged from 21.1-66.6 kgm•2 . The patients were admitted for upper

laparotomy under general anaesthesia after taking an informed consent from each patient. All patients (ASA I , IT ) were classified into two equal groups fifteen patients each:

Group 1: (norma l weight group or control group) include 15 non­ obese patients with a body mass index (BMI) less than 25kgm•2.Patients in this group were admitted for upper abdominal surgeries (e.g. vagotomy, gastrojejunostomy, haital hernia, exploration for the common bile duct, incisional hernia).

Group II: (morbidly obese group) include 15 morbidly obese patients with a body mass index (BMI) more than 35 kgm•2. Patients in this group were admitted for surgical management of obesity (e.g. gastroplasty).

Before arrival to operating room, every patient was pre-medicated with

0.01\kg\I.M of midazolam. On arrival to the operating room two intravenous cannulae were inserted and every patient was given fentanyl in a dose of 2)l /kg/body weight i.v, infusion of 0.9% sodium chloride solution was started and the volume of intraoperative fluid therapy was calculated on a ml/kg basis as 10 ml/kg body weight and titrated carefully against the haemodynamic status. An arterial canula was inserted in the radial artery and a central venous catheter, for volume management, was inserted in the right internal jugular vein.
