Effect of the degree of palatal coverage on retention of implant retained maxillary overdenture

Abstract


Eight patients were selected from the out-patient clinic of Prosthodontic Department, Faculty of Dentistry, Ain Shams University to compare the effect of degree of palatal coverage on the retention of implant retained maxillary overdenture.
The selected patients were having maxillary conventional complete dentures and mandibular implant overdenture.
A maxillary experimental complete denture was constructed for each patient to evaluate retention on it instead of the original denture of the patient.
Four 3 mm diameter, 12 mm length, one piece implants were installed in maxillary arch of each patient in areas of lateral incisors and first premolars to retain maxillary implant overdenture.
Retention was measured using digital force gauge four times for each patient; first time to the experimental conventional complete denture, second time to the experimental implant retained overdenture with complete palatal coverage, third time for the experimental implant retained overdenture with partial palatal coverage and fourth time to the palateless experimental implant retained overdenture.
Data was statistically analyzed and showed that retention of the implant retained overdenture with complete palatal coverage is the highest, retention of implant retained overdenture with partial palatal coverage was significantly higher than retention of palateless implant retained overdenture and there was no significant difference between both (implant retained overdenture with partial palatal coverage and palateless implant retained overdenture) and conventional complete denture.
From this study it was concluded that:
Retention of implant overdenture retained with four unsplinted implants with complete palatal coverage is the highestin comparison to the retention of conventional denture, implant retained overdenture with partial palatal coverage and palateless implant retained overdenture.
When four implants are used to retain maxillary implant overdenture the palatal portion of the overdenture can be partially or totally removed without impairmentthe degree of denture retention in comparison to the retentive value of the conventional complete denture.
