Abstract


Cancer pam management is still inadequate. It is mandatory to get rid of the causes of such inadequacy to improve pain management in cancer patients. Opioids constitute the mainstay of pharmacologic therapy of cancer pain. However, opioid use has its problems; especially in cancer patient. These problems include:
• modulation of the immune system and thus affection the rate of tumor growth,
• development of tolerance and dependence and thus limiting the usefulness of opioids; and
• emergence of opioid neurotoxicity.

Recently, approaches have been developed to reduce (or prevent at all) such problems e.g. using opioid immunopotentiators, using NMDA Antagonists to reduce tolerance and avoiding the risk factors responsible for opioid neurotoxicity.
