AUTONOMIC NERVOUS SYSTEM INVOLVEMENT IN RHEUMATOID ARTHRITIS

Abstract


Although peripheral and central nervous system involvement have been well recognized in patients with RA, ANS involvement has rarely been studied, and has shown conflicting results. Moreover, there is no general agreement on the prevalence of ANS involvement in RA.
This work aimed to study cardiovascular autonomic nervous functions in patients with RA compared with controls and to investigate fasting and after meal stimulated PP and its possible associations with autonomic dysfunction and disease activity.
This study was carried out in 60 RA patients (55 females and 5 males), they were recruited from the rheumatology and rehabilitation outpatients Clinic of Minia University Hospital, and I 0 controls (9 females and I male),
The exclusion criteria for the patients and controls were:

I. Age outside the range of 16 • 55 years.

2. Known somatic or psychiatric diseases.

3. Concomitant systemic connective tissues diseases or known primary neurological diseases.
4. Alcoholism.

5. Diabetes mellitus.


Rheumatoid arthritis patients were subjected to full history and clinical examination, laboratory investigations for CBC, ESR, RF, CRP, serum creatinin, plasma level of PP (fasting and after meal stimulated) and radiography.
Methods for measuring ANS involvement include: (I) Cardiovascular reflux tests by ECG.
(2) Frequency domain analysis by ambulatory ECG monitoring

(AEM).
