THE MYSTERY OF ALOPECIA AREATA A CLINICAL- IMMUNO HISTOCHEMICAL AND ULTRASTRUCTURAL STUDY

Abstract


Alopecia areata (AA) is a common disorder that produces a sudden patchy hair loss, characterized by increased percentage of telogen hair follicles. Genetic susceptibility and several factors including immunological factors have been suggested in its pathogenesis. However, there are conflicting findings regarding the role of cell mediated immunity and autoimmune phenomenon in the pathogenesis ofthis disorder.



Thirty patients were enrolled in the study (21 males and 9 females). Complete history taking, physical and dermatological examination were done for each patient. Immunohistochemical staining using immunoperoxidase technique for T-lymphocytes CD4, HLA-DR ICAM-1 were done on skin punch biopsies taken from the active edge of clinically diagnosed AA patients. Results were compared with 20 age matched controls. Also, electron microscopic examination were done to five patients. The biopsies were taken from the active edge and from the apparently normal
scalp of AA patients.

In this study, males outnumbered females. The age range was

8-47 years (mean 23.6 ± 9.34), and the mean duration (8.59 + 9.98) months. Difference between mean age of the patients and mean age at onset of the disease between progressive and stable groups were not statistically significant.
