Clinical Manifestations and Demographic Characteristics of HIV patients admitted to Abbasia Fever Hospital

Abstract


The present study was conducted in a cross sectioned manner on patients infected with HIV virus who are admitted to Abbasia Fever Hospital.
It included 100 consecutive patients admitted to Abassia fevers hospital from February 2015 to February 2016. All data about history, examination, investigations and treatment were taken.
The study results showed:
The male to female ratio of them is 1:4, with mean age of 36 years (range 5-65 years). Married patients were 65%. 25%of subjects were employed. Hand worker were 57% while only 18% were unemployed. Urban patients were (82%) while rural patients were 18%.
Abnormal Sexual Behavior was in76% of the patients, Intravenous drug abuse was 71%, Patients who had previous blood transfusion were 6%, Tattooing was in 6%, Patients who had previous surgical intervention were 4% and Positive Family history was 53%.
Chronic diarrhea was the most reported manifestations in the studied patients. It was seen in 30% of cases. The respiratory manifestations were seen in 28% of cases. Weight loss was present in 14% of HIV patients. 13% of the cases were accidentally discovered.
Lymphadenopathy was seen in 6% of cases. Dysphagia was seen in 5%. C.N.S infections were seen in 3% of the patients. Skin lesion was diagnosed in only 1% of the patients.
C.N.S infections were seen in 3% of the patients, in form of Cryptococcus meningitis and septic meningitis.
Pneumonia was seen in 19% 0f all cases. Pleural Effusion was seen in 6% of them. Lung abscess was determined in 2% of them. Bronchitis was seen in 1% of them. No abnormality detected was seen in 72% of all subjects and the median haemoglobin was found to be 9.8g/dl. (Hb< 10 gm/dl) was present in 59% of our study population. Blood picture of 87% of the anemic patients in our study showed normocytic normochromic anemia, while 13% of anemic patient had microcytic hypochromic anemia. Mean total leukocyte count was 3500 cells/mm3.
The mean of Absolute lymphocytic count was 720cells/mm3. (80%) of cases had absolute lymphocytic count (ALC) less than 1000 cells/mm3 (lymphopenia). The mean of Absolute Neutrophilic count (ANC) was 1365 cells/mm3. 87% of cases had Absolute Neutrophil count (ANC) less than1500 cells/mm3. Mean platelet count was 78.5. Patients with thrombocytopenia were 87%.
Splenomegaly was seen in 46% of HIV patients. Hepatomegaly was seen in 12% of HIV patients. Renal parenchymal disease was seen in 4%. Gall stone was seen in 4% of HIV patients. No abnormality detected was seen in 34% of all subjects.
