Abstract


SUMMARY
T
he treatment of breast cancer is multimodal and generally involves surgery, radiation and neoadjuvant and/or adjuvant systemic therapy.
Surgery is considered the primary treatment for breast cancer. Goals include complete resection of the primary tumor, with negative margins to reduce the risk of local recurrences, and pathologic staging of the tumor and axillary lymph nodes to provide necessary prognostic information. Several different types of operations are available.
The Breast Oncoplastic surgery is defined as a core component of the breast multidisciplinary team with sufficient experience to offer patients access to the full range of procedures encompassed by oncoplastic breast reconstructive surgery
Oncoplastic surgical operations are often long and frequently complex. As so often in surgical practice, careful patient selection (which factors in both co-morbidity and patient expectation) is the key to success.
Oncoplastic techniques extend the scope for breast conserving surgery by combining an extensive local excision of the breast parenchyma with a simultaneous reconstruction of the defect to avoid local deformity.
There is vast number of different surgical procedures and techniques which classifcied into volume displacement and volume replacement techniques.
The volume displacement techniques vary from the most simple incision pattern like batwing technique, round block technique, grisotti technique and hockey stick technique till the most complex technique of the therapeutic mammoplasty.
The volume replacement including the implant based reconstruction the autologous reconstruction like reconstruction with latissimus dorsi and TRAM flap. Each group has different indication and timing (immediate and delayed reconstruction).
The knowledge base of the oncoplastic surgeon should include a detailed understanding of the planning, selection and evaluation of the oncoplastic technique according to each patient.
Successful breast cancer treatment demands a collaborative team approach, requiring dynamic communication between a diverse group of specialists. In fact, a careful preoperative assessment helps to establish a comprehensive surgical plan that will achieve better results.
Oncoplastic surgery has begun to grow as a new subspecialty. As more surgeons adopt the oncoplastic concept, more techniques will be developed, advancing the subspecialty of breast surgical oncology.
