Abstract


Today overweight is one of the biggest health problems in the world, where it performs the background for many diseases.
After massive weight loss, patients suffer from excess skin that hangs from the torso, abdomen, and extremities is not only extremely unsightly but can be painful and susceptible to recurrent intertriginous infections.
Body contouring following massive weight loss is currently the fastest growing field within plastic surgery.
Massive weight loss patients are actually quite different in their presentations, according to their B.M.I. level at presentation, fat deposition patterns, and quality of skin-fat envelope.
Almost all patients present with more than one area of their body that they would like to improve. It is important to ascertain patient priorities and discuss staging scenarios.
Truncal skin excess extends circumferentially around the body. Integrated treatment of this complex deformity with lower body lift or belt lipectomy provides the optimum cosmetic result. However, many patients are reluctant to undergo such major procedures or are not candidates for such major procedures. Thus, a large subgroup of massive weight loss patients chooses to have only the anterior abdomen surgically treated.
Depending on the surgeon’s philosophy, the thighs may be dealt with early in the treatment plan or, better, left as the last area to be treated.
Postbariatric surgery patients present with loose and flaccid buttocks, and would be forced to choose between tighter but still flattened, empty buttocks or correction of the deflated, under- projecting buttocks.
Reconstructive surgery was performed on patients after massive weight loss was reached and maintained as monitored by the body mass index that has been stabilized for a period of at least 6 months.
All patients underwent extensive preoperative evaluation to detect both medically and psychologcally fit patients to undergo these extensive surgical interventions.
Complications were generally well tolerated by this patient population because of the often dramatic functional and aesthetic benefits that came with these procedures.
