Two-Dimensional Versus Five-Dimensional Ultrasound in Measurement of Nuchal Translucency Thickness Pilot Study

Abstract


One of the ultrasonographic markers used for trisomy 21 screening is nuchal translucency thickness (NT) that can be applied early in pregnancy.
Nuchal translucency is the sonographic appearance of subcutaneous accumulation of fluid behind the fetal neck in the first trimester of pregnancy. Only the fetal head and upper thorax should be included in the image for measurement of NT. A good sagittal section of the fetus, as for measurement of fetal crown–rump length, should be obtained and the NT should be measured with the fetus in the neutral position.
2D Ultrasound is basically an axial image and 3D Ultrasound is a volume and 4D Ultrasound is a volume with time and the fifth dimension is how do you bring a level of workflow into ultrasound, and it is basically bordering on the sense of automation. 5D technology is a form of automation where you go through and do a scan and you get the results autopopulated for you. Five-Dimensional Ultrasound included features like 5D-LB (fetal long-bone) and 5D-NT (nuchal translucency), 5D (fetal echo-cardiography), 5D-CNS (central nervous system)and 5D follicle monitor.
5D NT is a new software, which automatically identifies the midsagittal view and calculate the NT thickness using stored 3D data.
Aim of the study to evaluate the feasibility of five dimensional nuchal translucency (5D NT) ultrasound.
The current study was conducted in the Special Fetal Care Unit in Ain-Shams University Maternity Hospital in the period between March 2016 and March 2017. A total of 100 women were included in the study.
Two dimensional and five dimensional ultrasound examinations was performed with an S-W S80A Elite (Medison Co, Ltd, Seoul, Korea) ultrasound machine by Operator A (Dr. Rehab Mohammed) and Operator B (Dr. Monira Ali). Then, intra- and inter- observer variability were assessed.
Results:
In this study, 95% limits of agreement were calculated to range from – 0.97 to 0.83, i.e. for 95% of women, a NT thickness measurement by conventional 2D U/S would be between 0.27 mm less and 0.13 units greater than a measurement by 5D U/S.
Intraclass correlation coefficient, denoting the absolute agreement between measured values by both methods, was calculated to be 0.83, which is considered excellent correlation according to Cicchetti grading.
Mean difference between repeat measurements by the same observer was statistically significantly lower using 5D U/S compared to the conventional 2D U/S (0.016 vs 0.034 mm, p = 0.01). Also, 95% limits of
