MANAGEMENT OF PAROXYSMAL SUPRAVENTRICULAR TACHYCARDIAS IN PATIENTS WITH AND WITHOUT STRUCTURAL HEART DISEASES

Abstract
Supraventricular tachycardias arc one of dangerous arrhythmias that needs urgent management, the most common type of them is atrioventricular nodal reentry (60%), followed by pre-excitation type (30%).

The aim of this study is to compare between drug therapy (using either verapamil or adenosine) and electrical cardiovcrsion in ••cspcct to the p1•cscncc or absence of structural heart diseases.
The present study was done on 40-paticnts aged between 29-76 years presented with pa1•oxysmal attacks of supraventricular tachycardias, 19 of them were females and 21 were males. From those 20 patients were associated with organic heart diseases and 20 patients were not associated with heart diseases.
They arc classified into three groups, group (I) were 20 patients have been received vcrapamil, group (II) were 10 patients have been received DC cardioversion, and group (Ill) were 10 patients have been received adenosine.
Those patients were subjected to complete history taking and clinical examination as well as the following investigations: -
• 12-leads ECG before management followed by long lead II strip monitoring during and after management.
• Ck, CK-MB and LDH levels before management.
• Na• and K• levels before management.
• Serum creatinine and blood urea nitrogen levels before management.
• Echocm•diography after management.
Each patient has been monitored by blood pressure measurement, pulse rate and respiratory rates before, during and after management. Also, we observe for any complication occulTed during our management.
