Erectile Dysfunction in Patients with Heart Failure with Impaired Systolic Functions: Prevalence and Predisposing Factors

Abstract
SUMMARY
H
eart failure and erectile dysfunction have common epidemiologic associations, common risk factors and common underlying pathology. Furthermore, ED has been recognized as an early predictor of cardiovascular events, and might worsen with HF due to symptoms or cardiovascular medications. Very little is known on the prevalence of ED in the Egyptian community. Our study is the first study to estimate the prevalence of ED in Heart failure patients among Egyptian population.
Aim of the Work- To investigate the prevalence of erectile dysfunction in systolic heart failure patients and predisposing factors.
The study included- 400 patients diagnosed with heart failure during the period of May 2016 till January 2017 at Ain shams University hospitals clinic’s.
Patients was subjected to:
1) Detailed History with main focusing on:
A. Risk factors for Erectile dysfunction including: HTN, DM, Smoking, Dyslipidemia, peripheral neuropathy and IHD
B. Detailed drug history
C. Previous or on going participation in cardiac rehabilitation program.
D. Previous application of cardiac resynchronization therapy.
E. NYHA classification.
2) General examination including: Vital Data, signs of systemic or pulmonary congestion and BMI.
3) Laboratory tests including: Complete blood count, Kidney function tests and Lipid profile.
4) Quality of life (QoL) was assessed using Minnesota Living with Heart Failure Quality of Life Questionnaire.
5) 12 lead Electrocardiogram (for Rate and rhythm)
6) Transthoracic echocardiography for assessment of left ventricular ejection fraction.
7) Assessment of erectile dysfunction using International Index of Erectile Function (IIEF-5).
We found the prevalence of erectile dysfunction among heart failure patients was 82.2%.
On one hand- Age, HTN, Incomplete coronary revascularization, Peripheral neuropathy, high NYHA class, low Ejection fraction, low hemoglobin level, high serum creatinine, low creatinine clearance, AF, high BMI, high Minnesota score and the use of Digitalis, MRA, CRT all had a statistically significant relation with erectile dysfunction in heart failure population. Also the use of statins was associated with better erectile function.
