Factors Affecting Compliance toward Therapeutic Regimen for Patients with Diabetes Mellitus

Abstract
The promotion of therapeutic compliance is considered as an integral component of nursing care practice and patient healthcare. Noncompliance of the diabetic patients toward therapeutic regimen leads to accelerated development of complications and increased mortality. Clinical experience indicates that no improvement of metabolic control is possible without patients’ compliance to medications.
Aim of the Study
The aim of this study was to identify factors affecting compliance toward therapeutic regimen for patients with type 2 diabetes mellitus.
Research Question:
- What are the factors affecting compliance toward therapeutic regimen for patients with type 2 diabetes mellitus?
Research design:
A descriptive exploratory research design was used in this study.
Setting:
This study was conducted in diabetic outpatient clinic at Beni Suef University Hospital.
Study Subject:
A purposive sample was used. The total number of 114 patients was taken.
Tools for data collection:
The data were collected through using the following tools:
1. Patient interview questionnaire sheet: it was used to assess factors affecting patient's compliance toward therapeutic regimen (patient related factors, therapy-related factors, health care system factors, social and economic factors & disease factors). This part was adopted from (Jin, Sklar, Vernon & Li, 2008) and modified by the researcher. This tool were include the following:
(1) Patient related factors: it was include:
- Sociodemographic characteristics of patients: this part developed by researcher and used to assess the patient’s social and demographic characteristics as age, gender, residence, occupation, level of education, marital status, home care provider, having a device for measuring the blood sugar at house, monthly income for therapy costs, and smoking.
- Patient’s Knowledge: this part developed by the researcher and used to asses patient’s knowledge regarding diabetes mellitus and its therapeutic regimen (e.g., definition, signs& symptoms, complications, management and follow up…etc).
(2) Patient psychological factor: includes patient's attitude, beliefs, and motivation).
(3) Therapy-related factors: such as (route of administration, duration of the treatment period, medication side effects, requirements for drug storage…etc).
(4) Health care system factors: such as (lack of accessibility, long waiting time…etc).
(5) Social and economic factors: such as (cost and income, social support…etc).
(6) Disease factors: such as (disease symptoms, complications…etc).
(7) Smoking hazards.
2. Patient’s compliance assessment sheet: it was used to assess the patient compliance with medication, diet, exercise, blood-glucose testing, foot care, and smoking. This tool was divided into 2 parts:
A: Morisky Medication Adherence Scale (MMAS): it was used to assess the patient compliance toward medication for the patient with type II diabetes mellitus. This scale was adopted from (Morisky, 2010).
B: Diabetes Self-Care Activities Measure: this scale was adopted from (Toobert, Hampson & Glasgow, 2000) and used to assess the diabetes self care activities as: diet, exercise, blood-glucose testing, foot care, and smoking status.
Field Work:
The aim of the study and components of the tools were explained to the patients at the beginning of data collection. They were assured that the information would be confidential and used for research purposes only, as well as the research will assure maintaining anonymity and confidentiality of the study group data and they have the right to withdraw from the study at any time (verbal consent was taken from the patients).
The actual field work started at the beginning of October 2012 and was completely by the end of February 2013.
The researcher was available 3 days/week in diabetic outpatient clinics, at morning shift.
The researcher was interview from 3 to 5 patients daily.
Time taken to fill study tools was 45 minutes on individual basis to be filled depending on the degree of understanding and response of the patient.
