Abstract
etralogy of Fallot is one of the most common congenital cyanotic heart diseases, with an incidence of 10% of congenital heart disease (125).
Early primary repair tetralogy of Fallot in asymptomatic infants and young children has been advocated and performed with low operative mortality (126).
The question of whether to use a trans-annular patch arises in many cases, and this decision is now known to have far-reaching implications for long-term outcome. Recent publications continue to emphasize the detrimental effects of large trans-annular incisions. Trans-annular patch creates obligatory pulmonary regurgitation, and when this is long-standing and severe, important RV dysfunction will inevitably occur (127).
It is now believed that pulmonary regurgitation plays a major role in the development of right ventricular dysfunction, new surgical strategies focus on maximal preservation of right ventricular function by avoiding or minimizing ventriculotomy with TOF repair (123).
Total repair of TOF includes relief of RVOTO and closure of VSD. Several centers have reported satisfactory results with complete repair in neonates. However, as the long-
