Abstract


Tibial plateau fractures have always been a challenge for orthopaedic surgeons. These fractures may be difficult to manage and, although these lesions are relatively rare, the consequences of an inadequate treatment can be serious.
Review of literatures dealing with fracture tibial plateau in order to clarify the biomechanics of tibial plateau together with their mechanism of injury, some hints on anatomy of this region, fracture classification methods of diagnosis and current treatment options have been mentioned.
This is a comparative study which assess the significance of bone grafting in patients presented with split depressed tibial plateau fracture as regarding mainly fracture union, articular surface depression, condylar widening following open reduction and internal fixation of fracture tibial plateau using buttress plate and screws involving 30 patients ; 15 patients were managed with usage of bone graft and 15 were managed without usage of bone graft and followed up over a period at least 6 months, with evaluating their functional and radiographic outcome based on clinical examination and follow up plain and C.T. radiograph. Rasmussen knee scoring system was applied after six months postoperative for patients evaluation. This scoring tool consists of anatomical (radiological) and functional grading.
