EVALUATION OF THE PATTERN OF LOCAL RECURRENCE IN BLADDER CANCER PATIENTS FOLLOWING RADICAL CYSTECTOMY

Abstract


Bladder cancer has a very high incidence in Egypt constituting about 33% of all malignant tumours seen at the National Cancer Institute in Cairo. This is due to the endemicity of bilharzia! affection among the Egyptian rural population. Radical cystectomy remains the only therapeutic modality which gives reasonably acceptable long-term survival in bilharzia! bladder cancer, taking into account the relatively advanced stage of the disease on presentation. However, local recurrence following radical cystectomy accounts for most of the treatment failures and has remained an unsolved problem. This thesis focused on the problem of local recurrence following radical cystectomy. The study included one hundred and fifteen cases of bladder cancer. All our patients underwent radical cystectomy and were followed up meticulously till the end of the study period. We found that the peak incidence in our patients was in the fifth decade with a mean age of
51.97 years and a male to female ratio of 2.97:1. Most of our patients were of the P3 stage.
Squamous cell carcinoma was the most frequent pathological type seen in our patients constituting 56.52% of cases followed by transitional cell carcinoma 38.26% followed by adenocarcinoma 5.22%. The posterior wall was the most frequent site of tumour occurance followed by the lateral walls.
