Reduction Mammaplasty In Huge Breast Hypertropny

Abstract


This study dealt with the problem of breast hypertrophy in our part of the world, where huge breasts are predominant. Thirty-two cases were the target of the study from Feb. 1998 through Jan. 2000.
Six· techniques of reduction mammaplasty were used on sixty
four breasts in thirty two cases, inferior pedicled reduction was used in
12 cases, Mckissock reduction mammaplasty was used in 4, superior pedicled reduction in 6, central pedicled reduction in 5, Lejour with
liposuction in 3 and two with amputative breast reduction. In all cases, cup sizes, amount of fat removed, scars, nipple and areola sensation, symmetry, shape, physical problems were studied in each case. Individually, together with running an oral questionnaire for the cases. The follow up period was a minimum of six months for all cases, which reached to up to 23 months in three cases. Seventeen secondary procedures were done to correct scars, dog-ears, etc. with some cases performing more than one procedure. Superior pedicles gave better shape but with some risk on the viability of the nipple and areola, inferior pedicles were seen as the safest, easiest in all. Lejour technique and the central pedicle showed that they'd rather be used on less sized breasts. Mckissock is a safe procedure and remains a safe guard during inferior pedicle if the inferior pedicle was over trimmed or if preoperatively there was emptiness in segment 2 of the breast, where we would like to add some fullness. Complications were varying between dog-ears, wound dehiscence and slight asymmetry.
Breast reductions using the inverted T scan method remains still the best for achieving the best results in those patients with huge macromastia together with some modifications and the use of adjuvant procedures such as liposuction.
