CLINICAL PRESENTATION AND DIFFICULTIES IN MANAGEMENT OF MIRIZZI'S SYNDROME

Abstract


This clinical study aimed at studying the clinical presentation and difficulties in management ofMirizzi's syndrome.


The study included sixteen patients with calculous obstructive jaundice admitted to Gastro-enterology surgery unit in the Alexandria Main University Hospital. A detailed history taking, complete physical examination, laboratory and radiological investigation were done.


In all patients, a prolonged history of biliary symptoms is present, all were jaundiced in varying degrees and no pathognominic features on clinical examination.


The laboratory findings revealed hyper bilirubinaemia in all patients with elevated liver enzymes and alkaline phosphatase. Different imaging modalities were done for those patients in the form of ultrasonogtaphy, computed tomography, ERCP and MRCP.


Ultrasonography revealed cholelithiasis Ill all patients choledocholithiasis in two patients and proximal bile duct as well as IHBDS were dilated in all patients.


ERCP was done for fifteen patients and failed in one patients and the patients were classified according to ERCP findings into two types according to Me sherry classification.
