Study of the HCV status effect on soluble P- selectin levels as a marker of platelet Activation in hemodialysis patients

Abstract
P-selectin plays an essential role in the initial recruitment of leukocytes to the site of injury during inflammation and for platelet/leukocyte interactions When endothelial cells are activated by molecules such as histamine or thrombin during inflammation, P-selectin moves from an internal cell location to the endothelial cell surface. Hepatitis C virus (HCV) is one of the most common infections among hemodialysis(HD) patients. It continues to be exasperating problem in many HD centers, in spite of strictly following infection and quality control measures.
So, the present study evaluate the effect of (HCV) infection on soluble P-selectin levels as a marker of platelet activation in chronic hemodialysis patient and receiving (ESAs). The study recruited 40 HD patients divided into 2 groups : HCV negative HD patients comprising 20 patients on prevalent HD and HCV positive HD patients comprising 20 patients on prevalent HD . All participants were subjected to careful history taking, thorough clinical examination and laboratory investigations including p-selectin assessment.
In the present study, the duration of hemodialysis was significantly longer in HCV+ve than HCV-ve patients.
P-selectin assessment in the studied groups revealed a significantly higher p-selectin levels in all patients and the higher level was in HCV +ve group.
In the present study, there were no statistically significant relations between p-selectin levels and the demographic data in the studied HD patients. Also, there were no statistically significant differences between p-selectin levels in patients with various causes of renal failure.
In the present study, there were a statistically significant direct correlation between p-selectin levels and WBCs, urea, CRP, AST and ALT and inverse correlation with serum albumin reflecting the that increased p-selectin levels is related to deteriorated liver function with the more severe HCV infection.
