Abstract
Rheumatic fever (RF) is the consequence of a pharyngeal infection with group A streptococcus in a susceptible host, leading to autoimmune disease induced by antigen mimicry of human cardiac myosin with the streptococcal glycoprotein. Almost all cases of mitral stenosis are secondary to rheumatic fever and the consequent chronic rheumatic heart disease.
Balloon Mitral Valvuloplasty (BMV) has achieved good results in treatment of patients with mitral stenosis. Favorable results are obtained with good selection of patients suitable for BMV. With increasing experience and better selection of patient, the immediate results of the procedure have improved and the rate of complications declined.
There are many factors which may affect the result of BMV such as extrinsic factors related to the procedure and intrinsic factors related to patient age ,sex,rhythm,pathological affection of mitral valve,intial MVA ,presence of MR. The current hypothesis of CRHD is that fibrotic valvular deformity evolves through organization of acute inflammation induced by RF, with subsequent thickening and retraction of valvular leaflets, and with secondary damage resulting from turbulent flow induced by valvular dysfunction.
