Incidence and Management Of Hypospadias Fistula after Hypospadias Repair

Abstract


This study was carried upon 66 male patients admitted to Plastic

Surgery Department, Assiut University Hospital in the period from April

2000 to April 2002. The patients were classified into two groups, the first group (Group I) included 46 male patients presented with variable degrees of hypospadias, and the second group (Group II) included 20 male patients presented with hypospadias fistulas.

In group I, surgical procedures were done for hypospadias repair and follow up of the patients was done to calculate the incidence of hypospadias fistula which was 26.1 %. In relation to the age of the patients, the incidence of fistula was 75% in adults while in children it varied from I 0:5% to 42.9%. According to the site of hypospadias, the incidence of fistula was 20% in glandular, 16% in distal penile, 25% in mid-shaft and I 00% in proximal and perineal hypospadias. According to the method of repair of hypospadias, the incidence of fistula was 0% with MAGPI technique, 11.1% with Flip-flap, 45.5% with vascularized preputial flap and 50% with ventrolateral preputial flap. According to the duration of catheterization, the incidence of fistula was 50% in patients to whom the catheter was left for less than 7 days, 28% in patients to whom the catheter was left from 7 to I 0 days and 21.1% in patients to whom the catheter was left for more than I 0 days.

In group II, Repair of the fistula was done at least 6 month after the last surgical interference. The method for repair of the fistula was chosen according to the site, size and number of the fistula, and the avilability of healthy surrounding tissues. Follow up of the patients was done to detect recurrence of the fistula, which was 40%.
