Abstract


The selection of patients and of elbows best suited for total arthroplasty must be done very carefully.

Chronic synovitis of the elbow in rheumatoid arthritis tlwt has not responded to four to six months of optimal non­ operative management should be treated by synovectomy with or \Vithout. excision of the radial head, even in the a scnce of significant intra-articular X-ray changes. It gives satisfaction to 70% of patients for as long as five years after operation (Brumfield et al., 1981). When intra-articular damage is si!:,rnificant 'vvith pain, instability, and loss of flexion are indication for arthroplasty. Excision arthroplasty
\\'ith interposition of soft tissue has ben perfonned for nwny years, and Kimura and Yainio (1976) reported on 208 such operations. The joints were painless in 65% to 81°/o of cases depending on the type of resection, but weakness of extensor, and instability sometimes required external support, and residual pain was a problem in some-patients (Rydlwlm et al., 1984).
