Serum Level of Vitamin E in Children With Intractable Epilepsy on Ketogenic Diet

Abstract
his prospective study aimed to detect symptoms and signs of vitamin E deficiency, as well as measuring its serum level before commencing ketogenic diet and 3 months later in 20 patients having drug resistant epilepsy with mean age of 3.15±2.92 years. They were recruited from Pediatric Neurology and Nutrition outpatient Clinics, Children Hospital Ain Shams University hospital, during the period from January 2016 to June 2017. Four patients started classic ketogenic diet and 16 patients started MAD. All patients included in our study received multivitamin supplementation including vitamin E according to RDA in relation to age.
All subjects underwent full history taking, clinical examination and routine laboratory investigations (work up before KD), pelvi abdominal ultrasound and EEG in addition to serum vitamin E level before and after 3months of ketogenic diet therapy.
Regular outpatient visits were done at monthly intervals for a period of 3 months, during which, any reduction in seizure frequency or severity was recorded. The patient’s toleration to the diet was recorded, and any complication was treated accordingly.
Our study revealed a highly statistically significant reduction in seizure frequency per day and seizure severity assessed by Chalfont seizure severity scale after 3 months of KD therapy.
There was no significant difference in the mean z score of weight, lenght and BMI while comparing patients before and after 3 months of KD therapy.
Regarding lipid profile, there was a statistically significant increase in the mean serum levels of total cholesterol, triglycerides and LDL after 3 months of ketogenic diet therapy.
Although there was no significant difference in serum vitamin E levels between patients and controls before commencing KD therapy but they reached astatstically significant lower values after 3months.
There was a highly significant decrease in serum vitamin E levels after 3 months of KD therapy.
There was a negative significant correlation between serum vitamin E at baseline and 3 months later with Chalfont seizure severity score.
There was a positive significant correlation between serum vitamin E and HDL serum level.
CONCLUSION
