Association between Neutrophil/Lymphocyte Ratio and the Severity and Extent of Coronary Artery Disease in NSTEMI Patients

Abstract
Cardiovascular disease is the leading cause of death in western world.
One of its most insidious forms is coronary heart disease due to atherosclerosis.
Although many risk factors for coronary heart disease have been identified, they do not fully account for all cases of the disease.
Thus, the search is underway for additional biologic markers and especially inflammatory markers.
This research aimed to study the relation between Neutrophil / Lymphocyte ratio and the severity and extent of coronary artery disease in NSTEMI patients.
The study included one hundred and twenty (120) consecutive patients who presented to Dar El Shefa and Demerdash Hospitals in the period from June 2014 to December 2014 with NSTEMI.
Patients with the following were excluded from the study as these conditions affect the basal Neutrophil / Lymphocyte ratio:
1- Inflammatory diseases.
2- Bacterial infection.
3- Viral infection.
4- Steroid therapy.
5- Some forms of malignancies as lymphocytic leukemia.
All patients were subjected to the following:
1- Full history taking: name, age, medical history, risk factors (hypertension, diabetes, dyslipidemia and family history).
2- Full clinical examination: local and general examination
3- ECG was done to all patients to detect rate, rhythm, T-wave and ST-segment changes.
4- Venous sample was withdrawn from all patients on admission before starting medication and the following were done: Differential CBC, Cardiac enzymes and Troponine.
5- Echo was done to all patients to evaluate the ejection fraction, left ventricular dimensions, and the mitral valve.
6- Coronary angiography was done within 48 hours of admission via femoral arterial approach using 6F catheter with non-ionic, low-osmolar, iodinated contrast agent. Number of diseased vessels, and lesion sites were identified. Gensini scoring system was used to assess severity of coronary artery disease.
All data were collected and subjected to various statistical analysis.
