EARLY VITRECTOMY FOR DIABETIC VITREOUS HAEMORRHAGE IN TYPE 2 DIABETES MELLITUS

Abstract


The first vitrectomy for diabetic vitreous haemorrhage was reported by Machemer at the American Academy of Ophthalmology in 1970 (Machemer et al., 1971).

One of the important goals of vitrectomy is to restore vision and this goal is achieved by removal of vitreous haemorrhage with reattachment of the mac1,1la and removal of cataract.

The second equally important goal of surgery is to stabilize the diabetic neovascular process. The surgical objectives to accomplish this goal are removal of the vitreous scaffold, removal of surface fibrovascular proliferation and endophotocoagulation of ischaemic retina (Blankenship, 1981 and Blankenship, 1985).

The classical indications of vitrectomy for treatment of proliferative diabetic retinopathy are:
1. Traction macular detachment.

2. Vitreous haemorrhage.

3. Traction rhegmatogenous retinal detachment.


However, a group of new indications have been recognized for diabetic vitrectomy. These are:
