Ischemic mitral regurgitation Comparative study between different techniques of repair

Abstract


Back-ground: Ischemic mitral regurgitation (IMR) is a common complication after myocardial infarction (MI) of approximately (20%), which follows more frequently an inferior MI (38%) rather than an antero-septal one (10%). The optimal treatment of IMR is a matter of controversy however, much of this controversy stems from a lack of good data addressing this issue. The surgical approach for IMR has evolved from revascularization alone to an additional procedure; mitral valve repair or replacement. There were different techniques used in severe IMR repair.
Aim of the work: Comparing between the use of ring annuloplasty&Alfieri repair in severe IMR.
Patients and Methods: In this work we studied 40 patients who have IMR. Patients were divided into two groups: (Group I) 20 patients who underwent coronary artery bypass grafting (CABG) combined with mitral valve repair by ring annuloplasty.(Group II) 20 patients who underwent CABG combined with Alfieri repair.
Results: patients of both groups showed postoperative clinical improvement with low mortality percentage or serious morbidity. With the improvement being slightly apparent in group I than group II.
Conclusion: patients who were offered either approach have nearly results whatever the type of mitral valve repair used.
