Evaluation of Some low Molecular Weight Proteins as Markers of Early Renal Affection in Systemic lupus Erythematosus Patients

Renal affection is present in a large number of patients with Systemic Lupus Erythematosus (SLE), but only 65% develop apparent renal disease. A Percentage of these patients with latent nephropathy will develop clinically evident renal disease. The present work was undertaken in order to detect the prevalence of glomerular and proximal tubular dysfunction as assessed by Serum Cystatin C concentration, Urinary Retinol Binding Protein (RBP) and Urinary Alpha 1 microglobulin (A1M) execretion. The present work was conducted on thirty female SLE patients which were divided into two groups, twenty non renal and ten renal SLE patients. In addition ten healthy females as control group with matched ages were included. There was statistical significant increase in renal SLE group versus non-renal SLE group and control group regarding cystatin C, RBP: creatinine ratio and A1M: creatinine ratio. Also on comparing non-renal SLE group with control group there was the same statistical significance, the sensitivity of serum creatinine, serum cystatin C, RBP: creatinine ratio and A1M: creatinine ratio as derived from ROC curve was 80%, 90, 90 and 100% respectively, at a cut off values of 1.01mg/dl, 1.7gm/L, 0.66flg/mg creatinine and 33.31lg/mg creatinine respectively. From this study it was concluded that the efficacy of cystatin ?C as a screening test may even be superior to creatinine for early detection of impaired GFR and subsequent monitoring of renal disease in SLE patients. Also A1M. creatinine ratio can be assessed for screening and early diagnosis of tubular dysfunction.
