Iatrogenic Biliary strictures

Abstract:
Twenty patients suffering from iatrogenic bile duct strictures were
included in this study.
Roux-en-Y hepaticojejunostomy was employed in the surgical
management of (18) patients, hepaticoduodenostomy in one patient and
repeated stenting by ERCP treated one patient.


Results:
There were no operative mortality in this series. The commonest
complication was wound seroma and infection in (5) patients. Two patients developed subphrenic collection. Hepaticoduodenostomy was followed by restenosis one month later and treated by balloon dilatation.


Conclusion:
Management with preoperative cholangiography to delineate the
anatomy and placement of percutaneous biliary catheters, followed by surgical recostruction with Roux-en-Y hepaticojejunostomy, is associated with the most successful outcome.
