Study of Efficacy of Autologous Serum Therapy in Patients with Chronic Autoimmune Urticaria

Abstract
Chronic urticaria (CU) is defined as the recurrent occurrence of an itchy rash with or without angioedema persists on and off for six weeks or more. Prevalence of CU is believed to be 1 percent of general population but the real prevalence probably exceeds this figure. Prevalence of CU is likely to be present at sometime in about 25% of patients with urticaria.
Chronic urticaria may appear at any age although it occurs more frequently in adults and affects more women than men.
The pathogenesis of chronic urticaria has not yet been completely understood. The etiology of CU is heterogeneous. Autoimmune chronic urticaria nearly constitutes 45%. If no etiological factors are found in spite of extensive investigations the diagnosis is chronic idiopathic urticaria.
The diagnosis of CU is based primarily on the patient history and clinical examination which usually adequate to establish a diagnosis of chronic urticaria and Laboratory investigation are done to detect any underlying disorder when history and physical examination reveal no cause. The history itself can be regarded as the most valuable diagnostic tool in differentiating CIU from other types of urticarial.
Management of chronic urticaria can be non-pharmacological and pharmacological, and sometimes it is not possible to control the disease with antihistamines only, which are considered to be the mainstay of treatment. In severe cases of chronic urticaria, especially if autoimmunity has been proven, different modules of immunomodulation can be used: cyclosporine, cyclophosphamide, omalizumab, plasmapheresis, systemic corticosteroids, and immunoglobulin therapy. The use of such modules is limited by its high cost and/or adverse effect profile.
Autohemotherapy has been imperially used for decades in treatment of urticaria on no scientific background.
Aim of the study was to evaluate the efficacy of autologous serum therapy in patients with chronic autoimmune urticaria.
The study included 60 patients with CU recruited from the allergy outpatient clinic at Ain Shams University hospital. All patients were ASST positive.
