Abstract


Therapeutic plasma exchange is a treatment involving removal of the plasma from the blood and replacing it with fluid in order to remove a disease mediator circulating in the plasma. It is an exciting mode of treatment which is currently being used for a number of disease conditions. This therapy is very useful in treating immune related disorders by removing products of the immune system which can be contained in the plasma.


Therapeutic plasma exchange is accomplished most often by a centrifuge technique. Whole blood removed from the patient is collected in a spinning device, a bowl chamber or plastic bag. The higher density blood components (red blood cells) to the outside of the device, while the low density elements (the plasma) are displaced to the inside of the bowl. Separation may also be accomplished through a microporous that separates blood
elements by size.


Antecubital venipuncture is the most commonly used access for TPE, the large veins in the Antecubital fossa allow the necessary flow rates. For critically ill adults and for children, indwelling central or peripheral venous catheters are typically used.


Heparin and ACD (Anticoagulant Citrate Dextrose) are used as anticoagulant in TPE either in dependently or in combination.
