ASSESSMENT OF THE PORTAL VENOUS SYSTEM BY COLOR DOPPLER

Ahmed Maher Mohamed Arafa;

Abstract

Color Doppler Sonography has become a powerful diagnostic tool in evaluating the portal system. It can provide a great d,eal ofinfom1ation about the morphology and hemodynamics of portal hypertension.

It passively and automatically depicts blood flow in real time over the entire image or at least part of it (Grant eta!., 1989).

Color coded duplex sonography with its high negati ve predictive value, is available screening procedure for detect ing partial or complete, intra-hepatic portal vein thrombosis. If the portal vein is shown to be patent no further studies are required. In patients in whom color coded sonography shows no flow in the portal vein, other imaging studies as angiography or MRI should be performed to confirm the diagnosis of the portal vein thrombosis (Tessler et al., 1991).

On the basis of their location and direction of flow, porto-hepatic collateral can be differentiated from porto-systemic collateral. The presence of porto-hepatic collateral may be the first indication of portal thrombosis or stenosis (Grant eta/., 1990).
