The Role of IL28B Gene in Predicting HCV Recurrence in Liver Transplant Recipients

Abstract
BACKGROUND: IL28b has shown a great impact on the recurrence of HCV in post transplant recipients , donors and recipients IL28b was checked in both of them , as the HCV is a major problem world wide and liver transplantation is the last chance for such patients with liver failure and HCC , so a good selection of both candidates is a major issue to be consider to prevent further recurrence . METHODS: a total of 40 donors and recipients where included in this study in Ain shams centre of organ transplant patients with HCV related liver disease above 18 years where included , full blood tests and follow up for a year was conducted on both Donors and recipients , also a liver biopsy was done for those who developed signs of recurrence and after 6 month to all as a protocol . RESULTS: all recipients (100%) who had recurrence 14 patients were non-CC genotype , which was statistically significant, Donors with CC genotype showed corresponding recipients that developed recurrence significantly more frequent than recipients with donors of non-CC genotypes (12 patients 85.7% compared to 2 patients 14.3%).Most recipients who had non-CC donors (16 patients 69.2%) didn’t develop recurrence . Highest frequency of recurrence occurred in the combination of recipient donor genotyping non CC with CC. 12 patients 85.7% CONCLUSIONS: IL28b in both donors and recipients is a major predictor of HCV recurrence post LDLT .
